Aim: Cicatricial entropion secondary to trachoma is a common eyelid abnormality outside the West. In some patients, significant cicatricial entropion and trichiasis coexist. In this condition, simultaneous correction of the abnormal eyelid position and destruction of posteriorly misdirected eyelashes may be necessary. The aim of this study was to evaluate the efficacy of simultaneous Wies procedure and direct internal eyelash bulb extirpation for the management of cicatricial entropion associated with trichiasis. Patients and Methods: The combined procedure was performed on 65 eyelids of 53 patients with mild to moderate cicatricial entropion and true trichiasis, involving 51 upper eyelids and 14 lower eyelids. In each case, the number and location of eyelashes being treated was recorded prior to surgery to allow comparison at the time of follow-up 3 to 8 months later (mean 5 months). Results: Entropion was treated successfully in all patients. The overall success rate for the eye lash—ablating procedure was 91%. Shallow notching or localized madarosis was noted in 12% of patients at follow-up but this was cosmetically insignificant. Conclusion: Direct internal eyelash bulb extirpation is a safe and effective technique that can be performed in combination with the Wies procedure for the treatment of mild to moderate entropion and trichiasis.
