Aim: To compare the results of phaco-drainage and phacoemulsification. Patients and Methods: This prospective randomised study comprised 40 eyes of 37 patients with age-related cataract divided into 2 groups of 20 eyes that underwent phaco-drainage and 20 that underwent phacoemulsification. Postoperative evaluation was performed after 1 day, 1 week, and 1 and 3 months. Postoperative endothelial cell count and loss, best-corrected visual acuity, and postoperative complications were compared between the groups. Results: Mean best-corrected visual acuity in the eyes that underwent phacoemulsification (0.87 ± 0.09) and the eyes that underwent phaco-drainage (0.88 ± 0.11) was not significantly different between the groups postoperatively. Endothelial cell loss at 3 months was 6.35% ± 1.90% in the phacoemulsification group and 15.44% ± 2.36% in the phaco-drainage group (p < 0.05). Corneal wound oedema was found in 2 eyes in the phaco-drainage group (p < 0.05). Conclusions: When performed by experienced practitioners, phaco-drainage can be recommended as a safe alternative to phacoemulsification.
